¿Qué pasará cuando llegue la influenza A-H1N1 al Perú?

Como van las cosas con este asunto de la influenza A-H1N1 (nombre que reemplaza al de gripe porcina) en el mundo, la pregunta no es si esa gripe VA A LLEGAR O NO al Perú, sino CUÁNDO va a llegar.
Estoy reescribiendo el título y los primeros párrafos de este artículo mientras escucho en vivo y en directo por Radio Caracol de Colombia al ministro de Salud de ese país, quien está anunciando en vivo a las 7 de la mañana de este domingo, la confirmación del primer caso de influenza A-H1N1 en ese país. No es que yo acostumbre escuchar una radio colombiana a las 7 de la mañana de un domingo, lo que pasa es que he sido contactado por esa emisora para comentar el anuncio del ministro.
Terminada la larga entrevista, y luego de calmar un poco los ánimos de los periodistas radiales, me puse a pensar en cómo va a reaccionar la gente en el Perú cuando se confirme el primer caso de la influenza A-H1N1 en el país.
• ¿Será que el público ya “gastado” con la noticia lo tomará con calma?
• ¿Será que el pánico invadirá a la gente, y ciudadanos de pueblos muy alejados del lugar en el que se encuentre el primer caso empezarán a usar máscaras y a temblar de miedo?
• ¿Será que regiones enteras cerrarán escuelas, estadios y lugares de trabajo para “prevenir” casos de influenza? (hemos leído que increíblemente, sin tener un solo caso de la enfermedad en el Perú, las escuelas de Arequipa estarán cerradas por dos días esta semana).
• ¿Será que los políticos y los medios de comunicación aprovecharán el momento para “alzar” la noticia y obtener réditos de la situación?
• ¿Será que se presente una reacción imposible de predecir en este momento?
Con esa idea en mente, quisiera hoy día explicar cómo se hacen los estudios para saber si el virus influenza H1N1 llega a un país. Saber distinguir y catalogar un caso de sospechoso, probable o confirmado es sumamente importante y creo que todos (especialmente los políticos y la prensa) deberían conocer estos conceptos al dedillo.
Una de las preguntas más frecuentes del post anterior es la que se refiere al temor de los lectores de que cualquier dolorcito de garganta o estornudo que le empieza a una persona en Chiclayo, Lima o Arequipa pueda ser síntoma inicial de la influenza A-H1N1. En este artículo veremos por qué, en este momento, es muy difícil que esos síntomas se produzcan debido a la gripe H1N1.
Lo primero que se necesitaría para que eso suceda es que el virus, ahora llamado virus de la influenza A-H1N1, llegue al Perú y que se confirme un caso. Hasta el momento en que escribo este post, eso no ha sucedido.
¿Pero cómo se confirma un caso de influenza A-H1N1? Pues para que algo sea confirmado, es primero necesario que el caso sea considerado sospechoso y luego probable. Lo explicaremos en las siguientes líneas.
Antes de continuar, es muy importante primero definir a qué se llama enfermedad febril respiratoria aguda, esto debido a que no cualquier estornudo ni dolor de garganta puede indicar una influenza.
La enfermedad febril respiratoria aguda se define como
• Fiebre mayor de 37.7 (100 grados Fahrenheit) seguida por lo menos de uno de los siguientes síntomas:
• Catarro nasal o congestión nasal, dolor de garganta o tos.
De tal modo que antes de correr al consultorio o al hospital, tómese la temperatura y si tiene mas de 37.7, su catarro “ya tiene valor”.
Una vez comprobados esos síntomas, su caso está “listo para convertirse” en un caso sospechoso, el cual solo califica como tal si se cumplen una o más de las tres siguientes condiciones:
• Que dentro de los últimos 7 días, usted haya estado en contacto directo con una persona en quien ya se ha confirmado (véase después) la influenza A-H1N1. Al no haber ningún caso confirmado en el Perú, ninguna gripe del país es sospechosa.
• Que dentro de los últimos 7 días, usted haya viajado a una comunidad en la que se hayan presentado casos confirmados de influenza A-H1N1. De tal modo que si en la última semana usted solo ha viajado a Canta o Lunahuaná, no se preocupe.
• Que viva en una comunidad en la que se hayan presentado uno o más casos confirmados de influenza A-H1N1. El virus no está en el Perú, ninguna ciudad es sospechosa.

Imaginemos ahora que el virus llegue al Perú (como llegó ya a 18 países en el mundo), pues ahí sí empezaríamos ya a catalogar los casos como probables o confirmados.
Repito, solo después de que se compruebe un caso en el Perú estaríamos recién en condiciones de definir al caso probable como aquel en el que la persona tiene una enfermedad febril respiratoria aguda y resulta positiva para la prueba de laboratorio del virus de influenza A, y es negativa para H1 y H3 con la prueba RT-PCR. No quiero abundar en los detalles técnicos, basta decir que estas pruebas se hacen en etapas y demoran de 2 a 3 días en completarse.
Una vez que se cumpla ese requisito, recién el caso probable está listo para convertirse en un caso confirmado, el cual se define como el de la persona con enfermedad aguda febril respiratoria en quien la prueba de laboratorio específica para el nuevo virus H1N1 sale positiva en un laboratorio del CDC de Atlanta, prueba que demora dos a tres días más (al parecer el Perú ha comprado ya los reactivos que sirven para hacer esta prueba de confirmación). Recordemos que existen varias subtipos del virus influenza A-H1N1, el causante del susto actual es tan nuevo que recién se han desarrollado las pruebas para descubrirlo.
Y aquí es donde viene lo importante y esperamos que los políticos y los medios de comunicación sepan tratar adecuadamente cuando se compruebe el primer caso de influenza A-H1N1 en el Perú.
Si el caso se comprobara, por ejemplo, en una persona que vive en una casa de la cuadra 16 del jirón Huáscar en Jesús María, los casos sospechosos y probables solo van a estar en el entorno de esa persona infectada, es decir, solo en aquellos familiares, amigos y compañeros de trabajo que recibieron a menos de un metro de distancia los desconsiderados estornudos y toses del paciente. Serán precisamente todos esos contactos (previamente identificados por los especialistas epidemiólogos del Minsa) quienes podrán recibir los antivirales para impedir o disminuir las complicaciones de la infección. Solo ellos deberían preocuparse y de ningún modo los vecinos de la casa del lado ni mucho menos los del resto de la cuadra o aquellas personas que viven en la cuadra 15 o 17 del jirón Huáscar. Y eso porque el virus de la influenza A-H1N1 no viaja por el aire ni se mete por los resquicios de las puertas o ventanas de las casas, solo se contagia de manera directa y personal. Desde un punto de vista teórico, es posible que si ese paciente viajara en micros atestados de gente y estornudara o tosiera sin cubrirse, pudiera contagiar el virus a otras personas. También es posible que alguien se pueda contagiar por tocar algunos objetos previamente tocados por la persona enferma. De ahí la importancia, como dijimos en el artículo anterior, de aprender a estornudar y toser en público y de lavarse las manos continuamente.
Obviamente, a partir del caso inicial, es posible que se presenten otros casos, pero por el análisis de lo que se está viendo en otras partes del mundo que ya tienen casos confirmados, si bien el virus se contagia de persona a persona, parece que no es tan contagioso como se pensaba, de tal modo que se mantiene el concepto que queremos trasmitir en este artículo de que cada caso es "local".
En resumen, si apareciera el primer caso de influenza A-H1N1 en el Perú, todos los casos sospechosos de contagio de ese paciente estarían limitados a su entorno. Si aparece, como en nuestro hipotético caso en la cuadra 16 del jirón Huáscar en Jesús Maria, la gente de Surco, San Borja, San Juan de Miraflores, Huaral, Chiclayo, Iquitos, Arequipa o Tacna, no tendría nada que temer. El virus no viaja en nubes negras y no se desplaza a más de un metro de distancia de la persona enferma.
Es por eso que cuando se confirme el primer caso, no es que el virus “haya llegado al Perú”, y ni siquiera al barrio de la persona afectada, solo llegó a su casa y a su entorno inmediato. Todo el resto de gente debe estar tranquila, ni siquiera en México ha llegado a todos lados.
De ahí que es importante que aprendamos a toser y estornudar en público y a lavarnos las manos continuamente porque no sabemos lo que estamos tocando.
Y en lo referente a las máscaras, y considerando la ausencia del virus en el Perú, su uso puede considerarse más como una “monería” que como un arma de prevención en salud pública.
